The ordinary bureaucrats are not at fault, the entire system has been starved. The incentives are to just settle any disputes with a rap over the knuckles with a limp lettuce leaf. The Royal Commission into the Banks was a whitewash, they allowed very narrow criteria and only 12 months of evidence gathering including making the report. It was an inside job. Disgusting.
