In an expansion of your thoughts

(1) is this one BD or Calendar days under the act? If its a Calendar then it should be clear by a day
(2) This is true and i recognise the expiry extends to a decision, not the permit year. But if point 1 isn't an infraction then it would be reasonably expected that not meeting the permit year while extending it would be considered under the act, thus probably not an infraction?
(3) Really hard to say with any confidence why more information was requested. I do note the act allows you to move back and forth between each stage so unsure exactly what the concern was, or if there was any concern. With development application, the delegated authority can request more information in the decision stage if they don't think the officers recommendations are complete/ correct etc. Would of thought this would be similar so its hard to say with any confidence why it went backwards in the decision-making process
