I would suspect that they didn't debride the surface of the wound back to healthy looking tissue that bleeds. Instead of removing any dried fibrin exudate and taking off unhealthy tissue or infected tissue with a marginal blood supply by scraping and curetting and cutting away the rolled edges, they've just stuck BTM on top and it didn't integrate. So after a few weeks they had to do it again properly.
