BlastX wound gel and Bactisure irrigation lavage both contain 0.13% benzalkonium chloride. This is confirmed by the FDA 510k applications, provided as links in the previous sentence.
I don't believe the formulations of the sinusitis or otitis media applications are publicly available so they may have a different composition.
