    My wife is a neuropsychologist, and spends a fair bit of her time designing questionnaires for various diagnostic purposes. I asked her about the lack of a 'middle ground' option, and she said that it was very unusual in questionnaires such as this one. She thought it was likely because it is so short and without repitition (most similar quizes ask a set of similar questions, a number of times, but re-phrased).
