I can understand specialists earning $400k/pa.... I know many GPs and none earn much above $150k/pa...
Specialists people study for 10 years, train to be a specialist for another 5 years.... and this is fast-tracking. they can then charge what they think they're worth for the exclusivity of their skills and the degree in which they hold the lives of their clients in their hands. They take enormous risk and pay enormous professional liability insurance, as well as all the normal costs of running their own practice/business like any shop-keeper.
GP are in a similar boat and are constantly learning, often working 16 hr days, 24 hr on-call for several days/wk. not allowed to charge their actual worth due to domestic economics constraints.
Anyone can be a clear with very little training, aren't required to do more than their paid hours and do menial tasks.
egalitarian principles apply to the broad, general value of each individual being equal in law and enfranchisement or agency. its doesn't mean that each persons' effort is equal, or the worth of their individual contributions are equal.
