He knows she had a heart attack. How he (or anyone else) knows that a particular event that's mildly stressful caused it is anyone's guess. An awful lot of people have been under an awful lot more stress than either Kitching or Reynolds over the alleged Higgins rape without either dying or having a near death experience.
