same thing happened to me ...
Just called Nido Offer phone number and found out that form has not yet been received!!
If in doubt, call CommSec before closing of the offer and they can sort it out for you.
My NDOs are gone ... Such a shame it came to this. Another lesson learned not to trust management even they appear to be acting in your interest.
Ozgem, 62strats, Niu etc., thanks for all your valuable contributions on here. After all what's happened, your credibility has increased even more in my view.
