pig, I dont think there will be any issue for LVADs and class 4. Many people head straight to class 4 after an injury (MI, viral infection etc.) bypassing the initial 3 classes.
Personally, I think things like CRT and diuretic manufacturers are more likely to be in trouble. I think C-pulse is more likely to start being used earlier in the piece. Why live on constant diuretic support when you can just have a device placed which will assist and heal the heart.
I think we'll see C-Pulse start moving to gold standard for stage 2 even if the patient is tolerating medical management.
Its like a patient with OA, if they need to be on NSAIDs permanently, nowadays most surgeons will just give the patient a joint replacement... They wont wait until the patient can no longer walk as the outcomes are better if it put in a bit sooner than that.
