You are talking nonsense and throwing meaningless phrases around.
BHP had a death this year, Rio at least 2 in the last couple of years, FMG had 1 last year, etc. And that's just top of my head cases.
They were all actually work related accidents and I won't even go into derailed trains, automated machinery incidents and close calls they had.
You can't make dangerous work safe in reality with inspirational Facebook quotes, like you can in your head.
Accidents will always happen and you definitely can't control people's actions with 'safe workplace' sign in cafeteria.
