I'm about to complete the MOD entitlement and acceptance form...

    I'm about to complete the MOD entitlement and acceptance form for my shares entitlement. With regards to the top up offer, I'm planning to get some more if I can. Question is when I submit the form and pay the application monies do I pay the total amount including the proposed top up offer or is it just the shares entitlement?
