Just some context... The first failure (catheter tip breaking) is not great, but not life-threatening either. The second failure (significantly greater infusion rate) is definitely not great.
However, 2 reported events in 12 months is par for the course. Please search "infusion pump" on the same database, and you'll see these kinds of numbers for some major competitors;
Abbott - 4 events
Braun - 15 events
Baxter - 24 events
Carefusion - 69 events
etc
etc
