Dear Grant.
The largest cause of accident and or death was stupid people.

There own fault.

An example.
A fire on a morning that the snow was falling,
A person added fuel to the fire.
This fuel was a 5 gallon drum of petrol.

Safety ropes around a construction hole, that was about 60, maybe more feet deep, the bottom of which were steel rods, waiting for equipment to be lowered into place.

And a fool decided, he would cross the safety rope and walk on the other side.

A trolley coming down about a 60degree slope, carrying a load of about 10tonn or more, this person decided to walk in front of the trolley, even though being told not to, and as fate would have it, the trolley broke free, killing this person.

There is an endless list.

And of course accidents while driving.
Accident due to ice and snow and rain.

As a percentage I think low, when compaired to the road toll, there were thousands of workers, over say, 30years, even on a highway there would be accidents.

Another, a Dump truck laden with rocks for a Dam.
A safety guide person was there, 'for safety' to guide the truck to the edge, as to be able to place the rocks correctly, that is just don't dump the rocks over the edge.

The safety person, decided to see what a dump truck looked like going down over 100feet. The rest isn't nice.

All things considered, BEING THERE, and not reading stories from a Jurno, who was not there, by people sitting back in there nice soft chair, I truelly beleive that the results were fine, ok.

However any death is a great loss for those who love or work with that person.
