Just to clarify, excluding the upfront payments the suggestion is Mesoblast gets a minimum of, in the first year of $50m.  Royalties of 25% are only paid to Mesoblast when they exceed $50m.

After 5 years Mesoblast is paid $100m/annum plus any additional royalties amounting to more than $100m.

Using a very low figure of $50,000 per treatment (based on other treatments) that would not be hard to exceed, 2000 treatments per year given that CHF is supposed to affect 6.5m in the USA, not that all of those would be sick enough to treat with Revascor.  Still should be more than 2000 people per year.

OK, the general idea gets a TU from me, only quibble might be the upfront payment.
