There is nothing wrong with pointing the finger at the contractor if they are actually responsible for the problem, but where was the management taking responsibility for managing the contractor?

The biggest tell that something was rotten at DCN was the complete lie made in the half yearly that everything was fine when management knew there were serious problems back in Jan & Feb. If you want to go back further then you might get suspicious at the unrealistic production targets and the high capital spend.

One thing I think everyone missed (I can't recall anyone raising it) was a massive miss on the reserve grade. We all seem to take it for granted that reserves are a conservative estimate made by an independent CP of the resource. If we can't trust the reserve numbers what can we trust?
