I have a suggestion if your open to expanding on the knitting needle or kebab stick for scratching.
its the sweat that makes you it or the lack of air so perhaps give this a shot.
get a long straw, crush it flat, get a tad of pure alcohol or tea tree oil and put a few drops in the straw and then blow it down to the area you want to relieve.
also I would suggest to get a can of compressed air or use a compressor if you have one to aid in drying up your arm a bit or if your arm is sweating then blow some talcum powder down to also reduce some types of itch.
food for thought but just another tactic to try to save any insanity and give you more ability to keep focused.
of course, apply other additional ideas as they present to mix and match which one works best.
ps: there are some other creams also you could put down there which might be cortisol type ones or even savalon which will stop infection from scratch that leaves rash so again, experiment to your needs.
