Most important is the W 8BEN Form as without this form a payor may pay 30% witholding tax and equity gains will be exempt from the 30% withholding. Not sure what tax is applied as instructions did not specify so maybe the smarter SH can assist.

At first I thought it's in the too hard basket but with a quick DD seems to be straight forward.
