Have to agree with PafcU,sorry Jakedog, my thoughts are I do not know how the dam could be operated incorrectly. The only input is a slurry mix, is it the case that the pipe must me moved around to distribute the mix in the dam? If so then that may be operator error.
Probably in this mess because of GHD, should be suing them instead.
