there are plenty of active managers that underperform by more than 1.5% but don't go out of business.
there have been times when AFI has posted 10 yr figures that have equalled or bettered the index.
I don't know that there are many active managers that always do.
in their latest monthly report at least their NTA beat the index over the past 1 and 5 year periods ( including dividends & franking ).
