Instead of patting each other on the back and counting there bonuses management should have paid more attention to detail and basic common sense
Instead investors have lost millions and the project is delayed by a year

Who is patting each other on the back?
Firstly, they did send a trained nurse who have experience using the product to the US so the simple fact is protocol wasn't followed after s/he left.
Secondly, why wasn't the clinical trials in the US done in the same environment as in Australia and not the other way around (simply the protocol wasn't following 100%), and how hard is it to find a quiet consultation room? If you are sick, don't you want to be checked up in a quiet room so the doctor can focus 100%?
Thirdly, you don't have quiet period in a room at home or in doctors office (hence they are partnering with Telehealth consultants?
