Our collective thoughts are with you.
Why not give the antibiotics a go. See what happens in a week or so. In that time gather your near and dear and spend time with him and each other. Quite often, out of chaos and confusion comes sudden clarity and you will know how to proceed. Your confusion now is completely normal and part of the process of letting go and maybe saying goodbye.
Zipper is right, go with your heart but gather all around. You are not alone. If the antibiotics perk him up, well and good, life might resume it's gentle rhythm for a few more months or even years. And next time a similar situation presents itself you will be more prepared and less confused.
