Three already that I can recall from the last few years:

Man Monis - Lindt Cafe, Sydney, 2 dead
Farhad Jabar- NSW Police HQ, 1 dead
Numan Haider - Melbourne police station, 2 non-fatal stabbing injuries

In all three cases, the perpetrator was killed by police.

And no doubt the police have prevented many more with 24 hour surveillance of high risk suspects.
