who;s fault was it at manus island?

Its a difficult one to answer, as its a very sad event, that shouldnt have happened, and the question is could it have been avoided and if so how..

As we all sit in our armchairs and give our opinions and thoughts on what is right or wrong its far different than being at the facility, or being part of the team that has to make decisions.
Let me say it is not an easy task, and not everyone will be pleased no matter what policies are adopted..
So back to the unfortunate incident.. If the people looking after the detainees didnt incite the rioting, and the rioters themselves got out of hand, where is the blame laid?

Each and everyone on the Island, including police, guards and the detainees have responsibilities, when those are breached incidents can happen...
No easy solution... How can we learn from this, both sides need to learn, so this doesnt ever happen again
