Essentially pseudoprogression caused by an inflammatory response. The immune response is so aggressive that the number of inflammatory cells cause the tumour to look bigger on imaging or hotter on a PET scan.
It is a phenomenon that has started to occur with some of the newer treatments such as immunotherapy where the destruction of the cancer cells is caused by an immune response.
it causes a conundrum on imaging as initial scans can sometimes look worse when in fact it is a very good response.
It becomes clear on later scans as the inflammatory response declines when there is less tumour burden.
