He does not like spending any money on anything that does not have a direct way of making money. He can only see health of others in terms of how it can generate a direct profit when they pay for it. If they can't pay a profitable amount to get it then he does not see how providing it is of any use to anyone.
This isn't really some odd thing about Trump. It's a fairly common perspective in the US.
