    Lack of pressure on CEO to maintain momentum.
    The failure to think clearly.
    An absence of truth, honesty and realism.
    Over estimate project value and teams abilities.
    Laziness.
    Lack of focus and commitment.
    Repeated failure to meet targets.
    Lack of finance
    Hot air and hype over something that is just not that special.
    A CEO out of his depth.
