That really isnt a problem when planning brachytherapy. Each dose has a "field" and in theory you can be more precise with brachytherapy rather than SIRT which is dictated by arterial vascular supply. Multiple injections maybe required just like with SIRT where you may need several selective injections (+/- occlusion to stop gastric branches).
Skill level will be far less for OSL rather than SIRT - which is probably even more relevant for hepatic cancer as it is more prevalent in less developed countries... although if you can afford it you can afford a good hospital and a good radiologist !
