you seem to not understand the complexity of government in matters of health (and education).
funding goes to the states. National policies and national health programs are developed at federal level but they are not operational policies. States own what happens in their own state and also provide funding. Employment is a state responsibility. There are also public health networks funded federally which mediate some national programs and also interface with health providers such as clinics.
as to whether I’ve seen the mind numbing bureaucracy - yep but you can’t blame that on federal level alone either
so it makes no sense to point upwards only.
