"Optus outage" - The root cause was a default setting on the Cisco BGP router which disconnects when it receives a high number of BGP annoucements. Optus unaware of the default factory setting?
The problem was caused after a routine uprade at the Singtel peering exchange which provides the BGP routing info to the Optus BGP routers. Due to the BGP storm on the 7th (see graph) that ensured, 90 Optus Cisco routers disconnected from the network to protect themselves. The fix was to send tech's to the 90 sites to restart the routers.
