Can you please give examples of where management have let shareholders down "time and time again"?

I understand how losing money on a stock can turn someone into a blamer and a downramper, but apart from the one failed US trial last year, where else have management failed to deliver?

My experience is RAP management have consistently delivered to their stated timelines, and I'm confident they will continue to do so.
