You can contact +61 8 9389 8033 to see if you can obtain your individual Bpay details (which are on the form posted to you) and then you can just bpay the amount that you want to invest and they will work out the rest based on the amount you pay (ie 1 mil new shares and options = $10,000). Your don't have to complete the form if paying by Bpay. Hope that helps.
