Ended up talking to a chap named Tom, let him know the problem
"no problem sir i can sort that out for you blah blah." He informed me that I would receive an email confirming contact

Sure did received 6 all up stating

Only thing is 3 of them were W8-BENE forms, which I did not request the other 3 were account balances which at least state my holding and the date 10/2/16, I reckon that's the best I am going get from this AST
Can hardly wait for next year to do it all again that's providing  these dogs of shares are still around.
