24. Bileaflet Pericardial Patch Repair for Rheumatic Mitral Valve Disease

Objective: Surgical repair in the setting of rheumatic mitral valve disease is technically challenging and remains controversial. We report a case of mitral valve repair with bileaflet (decellularized) pericardial patch repair in a patient with a severe form of rheumatic mitral valve disease.
Methods: A 39 year old woman with a history of rheumatic mitral valve disease and dyspnoea on exertion was admitted for surgical mitral valve repair. Preoperative transthoracic echocardiography demonstrated mild to moderate mitral regurgitation and severe mitral stenosis with a mean pressure gradient of 12.6 mmHg and a mitral valve area of 1.1 cm2.
Surgical valve analysis revealed posteromedial commissural fusion; leaflet fibrosis and calcification at the A3 and P2 segments; fusion between the subvalvular apparatus and the fibrosed posterior mitral valve leaflet with restriction of the P1 and P2 segment. Surgical repair included release of the subvalvular apparatus and posteromedial commissurotomy. Furthermore, the severely fibrosed A3 segment of the anterior mitral valve leaflet was excised and replaced with a CardioCel (Admedus Inc., Perth, Australia) pericardial patch. Neochords were implanted to the free edge of the A3 and P3 segments. The fibrosed and calcified P2 segment of the posterior mitral valve leaflet was excised and replaced with a second CardioCel pericardial patch. Neochords were implanted to provide support to the neo-free edge of this segment. Ring annuloplasty was performed to complete the repair (Video 1).
Results: Predischarge echocardiography revealed no residual regurgitation and a mean pressure gradient of 3.2 mm Hg. The patient was discharged without any complications on postoperative day 6.
Conclusions: Pericardial patch techniques are progressively used to replace the diseased native valvular tissue in rheumatic mitral valve disease. This enables mitral valve repair even in severe forms of rheumatic disease. CardioCel pericardial patch can be used to perform leaflet repair in reconstructive valve surgery. Longterm follow-up is needed to study patch durability.
