My understanding is that the double refers to both the patient and the doctor not knowing whether they received a placebo or whatever they are testing.
I couldn't tell you what bias they are looking to control, but I'm not a doctor. I accept that doctors and researchers probably know what they are doing, and that they are almost certainly more qualified than I am.
