Following up on post market studies there has been some references recently to how some are used in regards to off label uses especially when for a medicine the original cohorts studied were restrictive especially with medical history and other treatments however post market are often much less restrictive better representing general population. Biggest problem is limited numbers involved.and small pharma only doing just enough and being budget limited not seeing upside in gaining more data.

Also a big push to eventually make it a legislated requirement that all studys forced to report and publish results so there is a larger database of failures so further funds can be better allocated and less duplication in the future especially highlighting poor design and using outdated comparitors . This would help all pharma better power their trials .
