Why are the trials so slow?

There are people in this forum who have expertise in the management of clinical trials. I direct this to them.

The end of the Phase 1 (Dose determination) DEP Docetaxel trial was announced two years ago (22 Sep 2017) with the immediate transition to a Phase 2 (Efficacy) trial. DEP Cabazitaxel was some time later and recently the DEP Irinotecan Phase 1 commenced.

As far as I know, we have not been advised of recruitment being complete for other than the above DEP Docetaxel Phase 1 trial.

The DEP Docetaxel Phase 2 trial is an "open label" trial meaning everyone gets to know what treatment is being given. We have been told the there are "encouraging signs of efficacy" against a number of cancers. The target number of patients is not very great and we have four centres involved.

My question for the experts is:

If the efficacy was obvious and substantial why would there not be a queue of oncologists trying to get their patients enrolled and treated. Why is it taking years to get a few dozen cases?
